COVID-19 outbreak and crew deaths
In April 2020, 19 crew members tested positive for COVID-19 after passenger disembarkation. The ship's head doctor and another crew member subsequently died.

Bardo National Museum attack involving shore-excursion passengers
Passengers traveling on a Costa Fascinosa shore excursion were among the tourists caught in the terrorist attack at Tunis's Bardo National Museum. Five Costa Fascinosa passengers were killed and eight were injured.
