    On December 17, 2025, during elevator renewal installation at Baiyulan Garden residential compound in Baoshan, Shanghai, the work platform's hook had not been load-tested and was not equipped with a safety lock or buffer; the hook broke, causing three people to fall with the platform to their deaths, with direct economic losses of about 5.36 million yuan. The investigation found that Shanghai Mitsubishi Elevator Installation and Maintenance Branch's safety management, safety briefings, and on-site monitoring were inadequate. The Shanghai Emergency Management Bureau fined the branch 2.5 million yuan and fined responsible person Li Hongjun about 486,000 yuan.
